MADE IN U.S.A. - TRADE MARK REG.U.S. Inside the lid cover, printed in black letters on a red background, is listed a chart for the photographer to use that that lists METRES (sic), FOCUS AT with a list of numbers; TO WORK AT and a list of numbers. Printed on the inside bottom of the tin it reads: FOCUS AT, FT. and a list of numbers; FT. IN., TO WORK AT, and a list of numbers. The lens is clear glass in a black rim. Printed in gold letters on the rim it reads: NO.5 - KODAK PORTRAIT ATTACHMENT - MADE IN U.S.A.
